List of Requirements

For Incoming Freshmen
1. Entrance Exam Result must be “qualified for admission”.
2. Original Grade 12 Report Card with applicant’s eligibility for admission to College duly signed by the school principal.
3. Original Certificate of Good Moral Character from high school (with school seal).
4. Original Birth Certificate from PSA/NSO.
5. Accomplished College Admission Form (should be accomplished online).
6. 2 pcs. 2 x 2 ID pictures (plain background).
7. Additional / Other Requirements:
A. For Junior High School completers of previous years (completion year is 2015 and prior) who did not enroll in any school after completion:
i. Certification from the high school principal stating that the Form 137 is still in the school file, and has not been forwarded to another school.
B. For students who attended Non-formal Education Accreditation and Equivalency or completers of Alternative Learning System Accreditation and Equivalency:
i. Certification of Rating with passing marks in all subjects with applicant’s eligibility for admission to College.
ii. Photocopy of diploma.
C. For Filipino students coming from schools abroad (except for Philippine schools under DepEd):
i. Authenticated Transcript of Records from senior high school (in lieu of number 2).
ii. Certificate of Completion in Grade 12 (senior high school or its equivalent).
iii. Recommendation from the school’s guidance counselor or principal (in lieu of number 3).
iv. Photocopy of applicant’s passport bio-page.
D. For Foreign Nationals:
i. Senior High School completer from high schools in the Philippines or high schools abroad under DepEd:
a. Alien Certificate of Registration (ACR) and Special Study Permit (SSP).
b. Photocopy of applicant’s passport bio-page.
c. Photocopy of birth certificate from the country of origin.
ii. Senior High School Completer from schools Abroad:
a. Scholastics Records or Transcript of Records duly authenticated by the Philippine Embassy in the applicant’s country of origin.
b. Certificate of Completion in Grade 12 (senior high school or equivalent).
c. Updated Alien Certificate of Registration (ACR) and Special Study Permit (SSP).
d. Photocopy of applicant’s passport bio-page.
e. Photocopy of birth certificate from the country of origin.

Temporary Enrollment Requirements for Incoming Grade 11 (in addition to the documents submitted during the entrance exam)
1. Original Birth Certificate from NSO/PSA.
2. Photocopy of NCAE Result.
3. Reservation Fee of Php 1,500.
Note: Failure to reserve your slot and pay the non-refundable reservation fee within the given period will automatically mean forfeiture of your slot which may be given to the other applicants.

Final Enrollment Requirements for Incoming Grade 11
1. Entrance Exam Result must be “qualified for admission”.
2. Original Grade 10 report card with applicant’s eligibility for admission to senior high school duly signed by the school principal.
3. Original Certificate of Good Moral Character from high school (with school seal).
4. Original Birth Certificate from NSO/PSA.
5. Accomplished Senior High School Admission Form (should be accomplished online).
6. 2 pcs. 2 x 2 I.D. pictures (plain background).
7. Photocopy of NCAE result.
8. Additional / Other Requirements:
A. For students who attended Non-Formal Education Accreditation and Equivalency or Completers of Alternative Learning System Accreditation and Equivalency:
iii. Certificate of Rating with passing marks in all subjects with applicant’s eligibility for admission to senior high school.
iv. Photocopy of diploma.
B. For Filipino students coming from schools abroad (except for Philippine schools under DepEd):
v. Authenticated Transcript of Records from Junior High School (in lieu of number 2).
vi. Certificate of Completion in Grade 10 (Junior High School or its Equivalent).
vii. Recommendation from the school’s guidance counselor or principal (in lieu of number 3).

a. Updated Alien Certificate of Registration (ACR) and Special Study Permit (SSP) issued by the Bureau of Immigration.
b. Photocopy of applicant’s passport bio-page.
c. Photocopy of birth certificate from the country of origin.
ii. Junior High School Completer from schools abroad:
a. Scholastics Records or Transcript of Records duly authenticated by the Philippine Embassy in the applicant’s country of origin.
b. Certificate of Completion in Grade 10 (Junior High School or Equivalent).
c. Updated Alien Certificate of Registration (ACR) and Special Study Permit (SSP) issued by the Bureau of Immigration.
d. Photocopy of applicant’s passport bio-page.
e. Photocopy of birth certificate from the country of origin.

For Transferees
1. For applicants from the Philippine Higher Education Institutions
A. Duly-accomplished application form.
B. Certified True Copy of Grades from previous school with grading system.
C. Course descriptions of subjects taken.
D. Transfer credentials / Honorable Dismissal duly issued by school last attended.
E. Original Certificate of Good Moral Character (with school seal).
F. Original Birth Certificate from NSO/PSA.
G. Duly-accomplished recommendation form.
H. 2 pcs. recent 2 x 2 I.D. pictures (plain background).
I. Entrance exam fee of Php 420.00.
2. For Applicants from Schools Abroad / Foreign Nationals
A. Duly-accomplished application form.
B. Scholastics Records or Transcript of Records duly authenticated by the Philippine Embassy in the applicant’s country of origin.
C. Updated Alien Certificate of Registration (ACR) and Study Permit issued by the Bureau of Immigration.
D. Photocopy of applicant’s passport bio-page.
E. Photocopy of Birth Certificate from the country of origin.
F. Entrance exam fee of Php 420.00.
Note: Crediting of Subjects will take at least five (5) working days upon submission of required documents.
